At PhysioWest, we believe quality care doesn’t always have to happen in a clinic.
Our home visit physiotherapy service brings expert, compassionate support directly to your home or care facility – helping you or your loved one stay active, independent, and confident.
Whether you’re:
- Recovering after surgery
- Adjusting after a hospital stay or medical change
- Living with a long-term condition
- Or simply feel more comfortable exercising and staying mobile at home
…our physiotherapists are here to support you to move well, live well, and keep doing the things that matter most.
Who Are Home Visit Physio Appointments For?
Home visit physiotherapy can be helpful for many people, including those who:
- Find it difficult or tiring to travel to a clinic
- Use a walking aid, wheelchair, or mobility scooter
- Are recovering after surgery or a recent hospital stay
- Live in residential aged care or supported accommodation
- Feel more confident practising exercises in their own environment
By coming to you, we can assess how you move in your own space and tailor therapy to your actual day-to-day life.
How Our Home Visit Physios Can Help

Rehab feels more achievable when your exercises are built into the layout and routines of your home.

The aim is to improve safety and confidence – not just in the clinic, but where you live and move every day.

We can support you to build confidence with:
- Showering and dressing
- Getting in and out of bed or chairs
- Cooking, light household tasks, and gardening

How Home Visits Work
We aim to make home-based physiotherapy simple and supportive from the first contact.
Step 1: Initial Assessment:
Your first appointment takes place at your home or care facility, at a time that suits you.
We will:
- Listen to your story and understand your goals
- Review your medical history and current needs
- Assess your mobility, balance, strength, and function
- Look at your home environment and how you move within it
For those referred post-hospital, we can often arrange this visit within 24–48 hours. We also welcome referrals and bookings for ongoing or preventative support.
Step 2: Tailored Treatment Plan:
Following the assessment, your physio will develop a personalised treatment plan designed around:
- Your health needs and safety
- Your goals – big and small
- Your home set-up and available equipment
- What feels realistic and achievable for you
This may include exercises, hands-on treatment (where appropriate), environmental recommendations, and strategies to support daily activities.
Step 3: Ongoing Support and Continuity of Care
You’ll continue to see the same PhysioWest team, so you always know who’s coming to the door.
We provide:
- Regular review and progression of your program
- Clear communication about your progress
- Updates for family, carers, or your wider healthcare team (with your consent)
We also support carers and family members with practical ideas for safe movement assistance and gentle exercises between visits.
